The documentation required for a smooth intake process

To provide you with the best possible care, we require specific documentation during the order placement and initial intake process. Below, you will find a detailed list of the documents we need, along with tips for taking successful photos of these documents.

1. Driver's License or Government-Issued Photo ID

A valid driver's license or government issued photo ID is required to verify your identity, name, and date of birth.

Details: Ensure the ID is current and shows your full name, date of birth, and a clear photo of you.

Tips:

  • Place your ID on a flat surface.

  • Use a well-lit area to avoid shadows.

  • Ensure all information is legible and in focus.

  • Avoid any glare by slightly angling the ID.

2. Front Facing Full Body Photo (for Weight Loss only)

In order for the medical team to review your eligibility, we require a photo of you showing your body type. The photo should be a front facing photo of you from the knees up. This photo helps the medical team to see your body type and make medication determinations and provide you with the most appropriate care.

Details: You can be fully clothed, but avoid overly baggy clothing. Ensure the photo is clear and taken in a well-lit area.

Tips:

  • Stand in a well-lit area, preferably facing natural light.

  • Ensure the photo shows your entire body from the knees up.

  • Wear fitted clothing that is not too loose.

  • Ensure the background is clear of distractions.

  • Use a stable surface or tripod for your camera to avoid blurry images.

Prior Prescription or Current Medication Label

Note: This is only required if you are not starting a medication at the starter dose, and is used to verify the current dosage of medication you are taking.

When placing your initial order of medication at amounts higher than the starting dosage, our medical team requires that you submit evidence that you have taken the chosen medication within the last several months. This can be a previous prescription, or the label of your current medication dosage.

Details: The photo should clearly show the medication name, dosage, and a date of issue within the last 3 months.

Tips:

  • Place the medication or prescription on a flat surface.

  • Ensure the label with the medication name, dosage, and date is clearly visible.

  • Use good lighting to avoid shadows and glare.

  • Hold the camera steady to ensure the photo is in focus.
