Complete Your Profile:
Start completing your profile with our onboarding wizard! This includes providing detailed information to ensure we can process your patient packet quickly and accurately.
Tips for a Smooth Signup
Clear Photos: Ensure all photos are clear and legible. This helps us verify your information quickly and accurately.
Accurate Information: Double-check that all the information you provide is correct. Any errors can delay the processing of your patient packet.

Information We Need from You
To send your patient packet to the provider for an asynchronous visit, we need to verify the following information:
Identity Verification:
ID Front and Back: Upload clear photos of the front and back of your ID. We need to verify your address, date of birth, and name.
Self Photo (Weight Loss only): Upload a full-body photo from the knees up. This helps the medical provider assess your need for GLP-1 medications. Your face does not need to be visible, but the photo should be clear.
Shipping and Billing Address:
Provide your shipping address and billing address. Upload a document if needed to verify your shipping address.
Weight and Height:
Enter your weight and height so we can calculate your BMI.
Pre-existing Conditions and Medications:
Inform us about any pre-existing conditions or other medications you are currently taking. This information is crucial for the provider to make an informed decision.
Previous Prescription (If Applicable):
If you have been on a weight loss medication within the past 2 months, upload a photo of your previous prescription. This can help expedite the process.

Finalizing Your Signup
Once all your information is verified, and your patient packet is sent to the provider, you will receive an alert in your dashboard. You can track the status of your prescription and shipment right from your account.
